How we work

Honest medicine. No upsell.

The private men's health space has some uncomfortable patterns — clinics that prescribe testosterone to nearly everyone who walks in, quick-script ED services that don't examine you, "wellness" panels that test 80 things to manufacture problems to treat. We don't work that way. Here's how we actually do this.

TRT only when genuinely indicated

We follow BSSM (British Society for Sexual Medicine) guidelines for testosterone deficiency. That means two paired morning blood tests, full hormone panel, exclusion of reversible causes (sleep, weight, opioid use, alcohol), and clear clinical symptoms. Borderline low testosterone with vague symptoms is usually not TRT territory — it's usually a sleep, exercise, weight or stress conversation. We'll have that conversation honestly.

ED is assessed properly, not just prescribed

Erectile dysfunction is often the first sign of underlying cardiovascular disease — the same blood vessels are involved. We assess cardiovascular risk, check blood pressure, review medications, ask about psychological factors, do bloods if indicated. Then we discuss treatment. Most men do well on PDE5 inhibitors; some need referral to a urologist for more advanced options. The right answer depends on the man, not the clinic's revenue model.

Conservative on PSA, honest about it

PSA testing has real benefits and real drawbacks — it can detect prostate cancer early, but also picks up benign conditions and slow-growing cancers that wouldn't have caused harm. We'll have a proper pre-test conversation about what a result might mean for you, then test if it makes sense. If the result is concerning, urgent urology referral the same day. We don't push PSA on everyone.

Shared care with your NHS GP, where appropriate

For ongoing treatments — TRT, BPH medications, depression treatment after men's-health screening — we write to your NHS GP requesting shared care. They continue NHS-priced prescriptions long-term following our recommendation. Most GPs accept this for licensed medications. For TRT specifically, some GPs prefer specialist endocrinology oversight — we'll refer if that's needed.

Discreet, judgement-free, on your timing

Men often delay sexual or urinary health concerns by years. We try to make this as easy as possible — book online (no need to explain to a receptionist), private clinic rooms, no awkward small talk. You can request a male doctor at booking. Conversations are confidential under GMC standards.

A standard consultation, an extended consultation with bloods on the same visit, and a follow-up rate within a month. Everything published on our price list. We don't sell "comprehensive optimisation packages" because most men don't need them. If you need ongoing treatment, you'll know exactly what it costs before agreeing.

Dr Bolat is our Clinical Director and the lead GP for men's health at MHW Clinic. GMC-registered, with over a decade of NHS experience in family medicine, urgent care and A&E, he brings a calm, methodical approach to men's health — the same approach he'd bring to a family member. Men's health is often as much about what you don't need treated as what you do; Dr Bolat is comfortable having that conversation.

For specialist input we refer to consultant urology (prostate, complex BPH, suspected cancer), endocrinology (complex testosterone or hormone cases), andrology (fertility workup), and plastic / general surgery for any surgical needs — either NHS or private depending on your preference and urgency.

Before you book.

Will I see a male doctor?

Dr Bolat is the lead GP for men's health at MHW Clinic and is male. You can request a male doctor at booking and we'll match you to Dr Bolat or another available male GP. If gender of doctor isn't important to you, we may also book you with a female GP — tell us your preference at booking.

I think I have low testosterone. Will you prescribe TRT?

Honestly — sometimes yes, often no. We follow British Society for Sexual Medicine (BSSM) guidelines, which require:

  • Clinical symptoms consistent with testosterone deficiency (low libido, ED, fatigue, mood, loss of muscle mass)
  • Two paired morning total testosterone tests (single test isn't enough — testosterone varies through the day and from week to week)
  • Full hormone panel including SHBG, LH/FSH, prolactin — to distinguish primary from secondary hypogonadism
  • Exclusion of reversible causes: untreated sleep apnoea, significant weight gain, opioid use, alcohol excess, depression, medications

If after this workup you have genuine testosterone deficiency, we'll discuss TRT properly — including the commitment (it's usually for life), the monitoring requirements, the fertility implications (TRT suppresses sperm production), and the cost. If your testosterone is borderline-low or normal-low with vague symptoms, we'll have a different conversation — usually about sleep, exercise, stress, weight, or alcohol — before reaching for a prescription.

I just want a quick ED prescription — do I need an appointment?

Yes — we don't issue ED prescriptions without a proper consultation, even for repeat prescriptions if you've never been assessed by us. Erectile dysfunction can be the first sign of cardiovascular disease, diabetes or hormonal problems, and the medications interact with several common drugs (nitrates, alpha-blockers). A 30-minute consultation includes proper assessment of all this; you typically leave with a prescription you can use straight away if appropriate.

If you want a script-only service without assessment, online pharmacies (LloydsDirect, Pharmacy2U, Boots Online Doctor) exist for this. We'd rather you saw us once properly, then continued with whatever's most convenient for you afterwards.

Do I need to fast for hormone blood tests?

For testosterone specifically — you need to be tested in the morning (ideally before 11am), as testosterone is at its peak in the morning and drops through the day. We don't routinely require fasting for testosterone, but if we're also checking cholesterol and glucose we'll ask you to fast for 10–12 hours beforehand. For confirmed testosterone deficiency diagnosis, we'll ask you to do two morning tests, ideally a few weeks apart.

How quickly can I be seen?

Most men's health appointments are available within the same week. For urgent concerns — a testicular lump, urinary retention, or a PSA result you're worried about — call us on 020 7916 0029 and we'll try to fit you in the same day.

What's the difference between standard and extended consultations?

The standard consultation is 30 minutes — appropriate for most concerns (ED assessment, urinary symptoms review, STI questions, lump examination, general men's health questions). You may leave with a treatment plan, prescription if appropriate, or a referral.

The extended consultation with bloods on the day is 45–60 minutes — appropriate for testosterone workup, ED with full cardiovascular assessment, or new-patient men's wellness review where we're looking at hormones, lipids, glucose, prostate and cardiovascular risk together. You receive a written summary report.

If you're not sure which is right for you, book the standard consultation and we'll upgrade only if needed — you won't be charged twice.

Will my NHS GP find out I came here?

Only if you want them to. For some treatments — ongoing TRT, BPH medications, ongoing antidepressant management — writing to your GP enables shared care so they can continue NHS-priced prescriptions. For one-off concerns (STI screening, an ED assessment with private treatment, a wellness review) you may prefer we don't write to your GP, and that's fine. We'll ask you at the appointment.

Will you do a prostate examination?

A digital rectal examination (DRE) is offered when clinically appropriate — usually if you have urinary symptoms, an abnormal PSA, or you're aged 50+ and want a thorough prostate check. It's a brief examination, done in a private clinic room, and you can decline if you're not comfortable. We'll always explain what's involved and why before doing it. For most men a PSA blood test plus symptom history gives most of the diagnostic information.

I'm worried about a testicular lump.

Please book sooner rather than later — call us on 020 7916 0029 and we'll try to see you the same day. Most testicular lumps are completely benign (cysts, varicoceles, hydroceles), but testicular cancer is one of the most common cancers in men aged 15–45 and is highly treatable when caught early.

If anything looks suspicious, we refer urgently to NHS urology via the 2-week-wait pathway (the fastest route for suspected cancer in the UK), and arrange a scrotal ultrasound the same week. We don't delay this.

Do you treat under-18s?

For men's health specifically (sexual health, hormonal concerns), our minimum is 16 years, and we follow Fraser/Gillick guidelines for under-18s on confidential consultation. For general adolescent health, our Paediatrics service sees up to 18 with Dr Velitchkova. For under-16s, NHS sexual health services are free, confidential and excellent — we'd point you there.
