Four common driver medicals.

If you've been told you need a medical for your licence renewal or application, here's what we cover. Each one is a specific regulatory form completed by a GMC-registered GP and signed on the day. If you're not sure which one you need, call us and we'll talk you through it.

TfL PHV / PCO Medical

Required for London black cab drivers, private hire drivers (Uber, Bolt, Addison Lee, etc.) and London licensed taxi drivers. TfL-specific medical form, completed and signed on the day.

DVSA ADI Medical

Required for Approved Driving Instructors (ADI) registration or renewal with the DVSA. Same standard as DVLA Group 2 (D4) but on the DVSA-specific form for instructor registration.

Group 1 Medical Review

Required for ordinary car licence (Group 1) holders who have a notifiable medical condition (diabetes treated with insulin, cardiac events, neurological conditions, sleep apnoea, vision changes), or for over-70 renewals where DVLA has requested a medical report.

Not on this list?

We don't currently offer maritime medicals (ENG1, ML5), aviation/pilot medicals (CAA), or railway safety-critical medicals, as these require examiners with specific external approvals. Before you book

What to bring and what happens.

Driver medicals are quick and straightforward when you turn up prepared. The single most important thing to bring is a full and current medical history — from your NHS GP if needed — because undisclosed conditions are the most common reason DVLA queries or rejects medical forms.

What to bring

  • Photo ID — passport, driving licence or biometric residence permit.
  • Glasses or contact lenses if worn — we'll test your vision wearing them. If you wear corrective lenses for driving, bring them on the day.
  • Full list of medications — prescription and over-the-counter. Include dose and frequency. Bring repeat prescription printouts if easiest.
  • Medical history from your NHS GP — a recent summary of your conditions, surgeries and ongoing diagnoses. The most important document to bring — ask your GP surgery for a "summary care record print-out" or "patient health summary" before booking.
  • Recent eye test certificate (if you have one) — from an optician within the last 2 years. Not essential, but helpful.
  • Specialist reports for any conditions — especially diabetes, cardiac events, neurological diagnoses, sleep apnoea, mental health treatment. Bring recent letters from your specialist or relevant test results.
  • Partial form if pre-filled — some employers / TfL send a partly-completed form with your details and licence number. Bring it; we'll complete the medical sections.

What happens in the appointment

  1. 01
    Medical history review (10–15 min)

    The GP will go through your medical history, current treatments, any conditions you've been diagnosed with, family history, and lifestyle factors that affect driving safety. Honest disclosure here protects you — an undisclosed condition that leads to an incident is far worse than a properly-documented one.

  2. 02
    Vision assessment

    Snellen chart for visual acuity. Visual field assessment by confrontation (or referral to an optometrist for formal perimetry if there's any concern). DVLA Group 2 has higher vision standards than Group 1 — we'll explain what's required.

  3. 03
    Physical examination

    BMI, blood pressure, heart and lungs auscultation, neurological check as required by the form. Quick, non-invasive.

  4. 04
    Form completion & sign-off

    Form completed, signed and given to you to send to the relevant regulator (or sent by post / online by you depending on the form). For complex cases requiring further information, we may need to follow up after the appointment — this is rare and we'll be clear if it's needed.

Honest disclosure protects you

Failing to disclose a notifiable condition on a DVLA medical isn't just a regulatory issue — it can void your insurance and create criminal liability if you have an incident. As your examining GP we're required to record what you tell us honestly. If you've been diagnosed with anything, declare it. Most conditions are compatible with driving when properly managed; we'll help you understand what needs disclosing and what doesn't.

Full pricing for all consultations, procedures and reports is published on our price list. We do not charge separate appointment fees on top of quoted prices, and all costs are confirmed before any test, procedure or report is started.

Common questions

Before you book.

Can I get a same-day appointment, including weekends?

Yes — we're open 7 days a week, 9am to 7pm, including Saturdays and Sundays, with same-day availability for driver medicals in most cases. Online booking shows live availability; for urgent appointments call 020 7916 0029.

How long does the appointment take?

Typically 20–30 minutes, depending on the medical type and the complexity of your history. ADI and D4 medicals take longer because the forms are longer. Allow up to 45 minutes if you have multiple conditions or detailed disclosures to discuss.

Will I get the form in my hand at the end?

Yes for most cases — the form is completed and signed during the appointment and given to you to submit to DVLA, TfL, DVSA or your employer. For complex cases that need follow-up information (e.g. a specialist letter we don't have yet), we may need to send the completed form by post / secure email within a few working days; we'll be clear about that at the appointment.

What conditions can fail my D4 medical?

The DVLA Group 2 standard is higher than Group 1. Some conditions that don't affect a normal driving licence can affect Group 2 entitlement — for example, certain heart conditions, untreated sleep apnoea, severe diabetes complications, certain neurological conditions, and significant vision impairment. The point of the medical is to identify any concerns; we don't "pass or fail" you — we record findings and the DVLA makes the final decision based on our report.

If we find something that may affect your application, we'll discuss it openly with you, document it accurately, and where appropriate suggest specialist input. Hiding conditions on the form is the most common reason for later DVLA action — honest disclosure is always the better path.

I'm not sure which medical I need.

Easy mistakes happen — especially confusing the TfL PHV medical (for London private hire / taxi work) with the DVLA D4 (for HGV / bus driving). Call us on 020 7916 0029 with your licence application or renewal letter and we'll tell you which form you need.

Can I bring a partly-completed form from my employer?

Yes — for some medicals (especially employer / DVSA forms) the first section is for you and your employer to complete with licence details, then the medical sections are for us. Bring the form with you and we'll complete the medical sections during the appointment.

Do you do home visits?

Not for driver medicals. The vision assessment and physical examination need to be done in a clinical setting with proper equipment (Snellen chart at calibrated distance, properly-zeroed BP equipment, etc.). The clinic is centrally located in Whitechapel, 5 minutes from Whitechapel and Aldgate East stations.

Do you do maritime (ENG1, ML5) or aviation medicals?

No — maritime medicals require an MCA-approved doctor (ENG1) or MCA-recognised general practitioner (ML5), and aviation medicals require a CAA-approved Aeromedical Examiner (AME). These approvals are external to GMC registration. Try the MCA approved doctor list for maritime, or CAA for aviation.

What if you find something that affects my licence?

We document what we find on the form — we don't suppress findings, and we don't sign documents that aren't truthful. If we find a concern, we'll discuss it with you, explain the implications, and where appropriate suggest you see a specialist (e.g. cardiology, sleep clinic, ophthalmology) before re-applying. The cost of the medical is not refundable if findings affect your application — you're paying for the medical examination, not for a guaranteed positive outcome.

Can my employer pay directly?

For multiple-driver bookings (haulage companies, taxi operators, driving schools), we offer corporate invoicing — email info@mhwclinic.co.uk with your requirements. For individuals being reimbursed, we provide a detailed receipt at the appointment that you can claim back.
